Please complete this prospective client form to get started. We will email you back as soon as possible with some paperwork and scheduling times for an initial appointment. We accept BCBS, Aetna, and private pay. We also have interns for low-fee options.

Please know we no longer take active crisis clients. If you are engaging in suicidal behaviors or have a recent attempt or psychiatric hospitalization, we require you to complete an higher level of care first. This is either a partial hospitalization program (PHP) or intensive outpatient program (IOP) program. Please let us know if you need referrals, prior to scheduling outpatient therapy with us.
